*I am currently on anti-cancer and heart medication
2. ACCURACY & RELIABILITY OF THE DEVICE
I request disclosure of the make, model, and current calibration
status of the testing device being used, along with its known margin
of error and any substances or conditions known to produce a
false-positive result and any toxic agents contained within the testing proceedure
3. CHAIN OF CUSTODY & PROCEDURAL COMPLIANCE
I request assurance that all samples collected will be handled in
strict accordance with the required chain of custody protocols, and
that I will be provided with a copy of all results and documentation. In addition I request
confirmation that the test will be used only for the purpose of drug testing and not for any other
undisclosed purpose including but not limited to DNA sample collection.

4. RIGHT TO INDEPENDENT TESTING
I request clarification of my right to seek an independent confirmatory
test, should the initial result be positive or inconclusive.
5. LEGAL BASIS FOR THE TEST
I request the specific legislative authority under which this test is
being administered (e.g., Land Transport Act 1998 or relevant
amendment), and confirmation that you have followed all procedural requirements under
that legislation and have been fully educated to administer this test and comprehensively
briefed on all Health & Safety protocols regarding the administering of this test.
I wish to make it unequivocally clear that I am not obstructing or
refusing this process. I am exercising my right to be fully informed
before consenting, in the interest of protecting my health, my legal
rights, and the integrity of any result obtained. I am willing to cooperate fully once the above
clarifications have been provided and noted on the record.
Yours sincerely,
