📌 Step 2: Certify Your Affidavit
Once completed, ensure a Registrar or Justice of the Peace (JP) witnesses your signature.
📌 Step 3: Submit Your Affidavit
Once signed and certified, scan or take a clear photo and upload it below.

Please upload a scanned copy of your signed and certified affidavit (PDF, JPG, or PNG format only).
Only you can see the file you upload. Admin will securely receive it.
